Cost & Fees

Comprehensive Adult Evaluation:
$3,000, based on time and complexity -
Materials Fee:
$100 non-refundable materials fee

What to expect
Complete the contact form and a member of our scheduling team will reach out to gather information and discuss provider availability.
An estimate of benefits/cost will be sent to you prior to your testing appointment. Estimated patient costs are required to be paid in full on testing day.
Our out of network fees range from $1500-$2000 per evaluation.
You will be scheduled for three appointments
Intake – 1 hour where your provider will gather information about your history and presenting concerns and obtain contacts for collateral informants.
Your provider will meet with Dr. Ford our assessment director to choose the appropriate tests for your area of concern.
Testing– 2-3 hours is set aside to complete a battery of tests
Your provider will score and complete a write up of your test results. Dr. Ford will approve the final diagnosis and write up prior to your feedback session.
Feedback– 1 hour meeting to discuss the results and recommendations. Your report will be sent to you shortly after this meeting.
